Our Children Succeed Initiative

What are the program services?

The Youth Transition Program services assist young adults moving towards independent living. These services include, but are not limited to:

  • Independent living skills
  • Shopping
  • Food preparation
  • Housekeeping
  • Making appointments
  • Money management
  • Establishing transportation  (driver's license, public transportation, volunteer drivers)
  • Mental health management
  • Managing medication
  • Understanding mental illness
  • Increasing skills (coping, anger management, social)
  • Attending psychological education groups
  • Acquiring and maintaining housing
  • Crisis assistance
  • Employment
  • Job readiness assessment
  • Skills training
  • Finding and maintaining community or sheltered employment
  • Education
  • High school diploma or general education diploma (GED)
  • Career assessment
  • Higher education option
  • Skills building
  • Communication
  • Relationships
  • Boundaries
  • Problem-solving
  • Conflict resolution
  • Benefit assistance
  • Applying for state and federal program benefits (Supplemental Security Income, Social Security Disability Income,  Medical Assistance, Medicare, HUD,  WIC and Bridges.)
